1. Click proceed to online forms

2. Create or login to your parent account

3. Enter your student's information as prompted to add your student to your account. If you have multiple children playing sports, choose "Click here to add a new student" and follow same instructions for each child-athlete.

4. Once student(s) is/are found in the system, click "Start Forms" tab

5. Click view to access the forms and to see your progress on completed and incomplete forms. There are four forms that need to be completed, including Next Play Mentality Contract, Physical upload form including both the physician signed physical form AND the Medical History form, TAPPS Student Profile, and TAPPS Signature Page. The TAPPS signature page will include all the other necessary documents (such as sudden cardiac arrest and concussion awareness). Please do not bring in any hard copies of forms, but keep them and upload them as a PDF.

6. Follow prompts on each form and insure they are marked as complete on the "view" page.
